| In collaboration with a physician, assumes care management of a specific patient population. Following established nursing standards, procedures and practices, and working within the State of Florida Board of Nursing guidelines gives specific patient care directions to nursing and other staff. |
Provide direct patient care, evaluates outcomes, consults with other specialists, and adjusts nursing care processes as indicated to ensure optimal patient care.
Provides case management services.
Performs histories and physicals. Records initial histories, assesses patients’ conditions and develops individual care plans for patients. Monitors and dictates/records patient data in medical records to include patient call backs, assessments, pre/post operative orders and discharge summaries.
Orders appropriate diagnostic tests to evaluate and/or monitor patient’s progress. Provides ongoing medication management.
Monitors and ensures accuracy of recording on patient medical records. Maintains all required reports, records, statistics, etc.
Completes daily rounds when necessary on all in-patients and documents plan of care. Manages discharge of patients with appropriate disposition, follow-up planning and therapy management.
Enhances professional growth and development through participation in educational programs current literature, in-service meetings, and workshops.
Demonstrates knowledge and skills necessary to provide age appropriate interventions ranging from adolescents to geriatric patients.
Complies with all safety regulations, policies and procedures as defined by Bay Medical-Sacred Heart, including completion of medical records. Ability to float and perform Midwife duties in multiple office(s) and hospital settings.
After hours “on call” as needed.
Involvement with marketing, community education and promotion.
Performs other duties as assigned.
